One group whose mission is to do exactly as you state, "to make
physicians/HCPs more fully realize that health care is truly a
collaborative effort for the best results. And including patients in that
team would well emphasize that." is the Society for Participatory Medicine

Fascinating and active group of providers, patients, IT people,
visionaries--just as in this group,

The other thing that it would do is to make physicians/HCPs more fully
