Comment period on AHRQ Prototype Patient Safety Reporting System

I am not so sure that lumping the two together does not make sense. A diagnosis is essentially always linked to medical advice, even if the advice is that no additional action is advised. Similarly, advice in the absence of a diagnosis infers diagnostic information.

What jumps out at me is why are they lumping 'diagnosis' and 'medical
advice' together? Surely it would make sense to separate them as medical
advice is so broad.
