failure analysis tactics and cognitive bias

They mention it often in reviews (as in this year's article published in
Annual review of public health: Reducing Hospital Errors: Interventions
that Build Safety Culture or doctoral theses  - e.g.,
http://scholarcommons.usf.edu/cgi/viewcontent.cgi?article=4249&context=etd),
but I don't know anyone who actually used it for cognitive biases.

And some people believe that success of these methods for clinical decision
support (and other dynamic, complex, human-driven systems) has limited
potential
