A tip of the hat David! 
I am not a doctor, but a 24/7 care giver to a misdiagnosis patient who walked into a university, and left 15 days latter in an ambulance, was carried home a total of 61 days latter, never to care for her own human needs again, let alone walk.

listening to the patient, MRI ordering and differential diagnosis basics fall under the same "basics" that are missed.

So here is a twist to this discussion -- is it the stethoscope that is inaccurate or have we become less accurate in using it?  Here in the States, we can quickly move from "I hear something" to obtaining some form of imaging study whether it be an x-ray or echo.  We are also challenged by the growing body habitus of our population.
