We announced at the 2013 DEM meeting that our proposal has been approved by the Executive Committee of the IOM's National Research Council Governing Board and enthusiastically endorsed by IOM President Harvey Feinberg and several leading people in the patient safety community. The IOM's "To Err is Human" in 1999 was the inspiration for the current patient safety movement, and it is our hope that this same type of impact could be realized for diagnostic errors by specifically focusing on this issue. In addition to summarizing the current state of understanding concerning diagnostic error, the report is expected to outline the current gaps in this knowledge, and actions the many different stakeholders could consider to close these gaps and reduce the risk of diagnostic error.

Any suggestions on additional funding sources would be greatly appreciated; we would all love to see this project started as soon as possible.
