Patient Safety Metrics

In addition to the usual ones, my favorite metric at our hospital is
number of reported events.  One of our hospital's key performance
metrics this year is to have at least 10,000 safety events reported. 
Each department is tracked for the number of events per month.  Our
residency reports 40-60 per month. We also do an annual departmental
survey asking about the safety culture -- with questions like "do you
feel that problems that you raise will be effectively addressed".

We measure a number of processes – fall risk assessments, compliance
with scanning medications, surgical checklist compliance, etc. 
We are wondering what are some patient safety effectiveness metrics
that others utilize that are more direct measures of patient safety?
