What is next in diagnostic correctness?

If the criteria for diagnoses are sometimes imprecise or incorrect, the diagnostic words do not convey the diagnosis concisely to all, the words that describe the diagnosis are sometimes insensitive to patients, the diagnoses are are frequently not understood by patients (after all the language of medicine developed with an overtone of secrecy), and with all this confusion itself possibly leading to mis-diagnoses, what should the next step be?

Most/many conditions and diagnoses were described and written about by one person to a small group of people many years ago without reference to the big medical picture and the needs of patients and healthcare professionals today.

Can ImproveDX play a part? Is the challenge too big? Is this something for the Institute of Medicine?
