John Ely, Mark Graber and Pat Croskerry have a list which is updated routinely. I have the most recent as of about 10 days ago if anyone would like it.
The checklists are modified from Ely JW, Graber ML, Croskerry P. Checklists to reduce diagnostic errors. Acad Med. 2011 Mar;86(3):307-13.
