So Manoj,
              By your astute reckoning that retrospective analysis of
diagnostic errors may lead to over-estimation, a "hindsight bias"- in this
instance bias introduced occurring in search of bias. Hindsight bias often
occurs with "black swan" events(like 9-11), when in retrospect it was
obvious we should have known("all the warning signs were there"- is often
murmured quotation). In essence, history happens forward, but is examined
backwards(in retrospect)
