I suspect many of you have already seen this but it has not been posted
here.  

 

The Bob Berenson wrote a report on diagnostic errors for the Urban
Institute, funded by RWJ.  It is very well done and should be carefully read
by everyone interested in the topic. 

 

I like the fact that they recognize how hard it is to measure dx error and
therefor propose a range of policy options that go beyond simply trying to
measure and hold organizations accountable for reducing specific dx errors.
