I believe you are looking for patient-provider communication and diagnosis link within the broader context of the patient-provider clinical encounter. If so, you might find some of our papers web-linked below useful. The papers look at this from the general perspective of diagnostic error and communication breakdowns (within taking a history, exam etc) and might not look at this at a granular level you describe below.
