Diagnostic Error as Major Issue in Healthcare reported in ECRI and ASHRM Today

I do not have in-depth experience with HC insurers in this respect but I do have experience with the larger insurance companies that insure global manufacturing holdings.

In the manufacturing industry, most large insurers offer premium discounts for evidence of effective proactive/preventive efforts to reduce excessive risks and thus the potential of their consequences.

On the flip side, my experience with manufacturing industry investigators was their primary concern was to minimize the payout of a claim, rather than understand the latent root causes (system root causes) that contributed to the undesirable outcome.

To me, this was always a short sighted approach, because they did not remove the roots from weeds, so the weeds will continue to grow in their system.  This means the risk potential for recurrence is great and opens up the pathway for another claim.

Sadly, hospitals and hospital risk managers have been mostly absent from discussion or action on diagnostic error in medicine.
There are signs this may be changing.
Today ECRI published a short article titled,“The Difficulties in Defining and Preventing Diagnostic Errors”.

The ECRI article references another article in the July issue of Journal of Healthcare Risk Management, a publication of ASHRM, the Association of Healthcare Risk Managers, which came into being with the assistance of the American Hospital Association.
