Diagnostic Error as Major Issue in Healthcare reported in ECRI and ASHRM Today

You would think that there would be discounts if that event leading to a claim did not occur again, or penalties if it did.

Is it a problem with accurate data collection?  Is it that there is poor correlation between and events and claims?

I  still do not fully understand.
