Disclosing pharmacy errors

Dear List,

When a medication error takes place at a hospital pharmacy, who is responsible for disclosing that error to the patient and/or substitute decision maker?  For purposes of this hypothetical question, assume that there was no error on the part of the physician who ordered the medication.  What I am getting at are the cultural norms around the issue of who should be responsible for disclosing the error.  The approach of radiologists to disclosing error may be instructive here.
