TV Interview with Mark Graber and Sonia Millsom of Best Doctors

I was thrilled to hear the good comments on the NYC Larry Parks Show, the
about all our good efforts, and took special pride in hearing praise of the
Patient ToolKit.

I have just converted it to a form which can be filled out online, which
was a repeated request.  I may also work on an update of the form,
structured for the patient with an ongoing problem, chronic condition, as
that would shift the conversation.

Please disseminate this form freely, and provide any feedback, whether your
own or from others.
