an interesting article on telemedicine

There is a lot of activity in telemedicine.  From the viewpoint of SIDM, I 
think we have some obligation to try to critically evaluate if 
telemedicine is associated with more diagnostic errors and if so what is 
the mechanism.  There are many potential reasons why diagnosis may be 
wrong in a telemedicine program such as not having the medical record (not 
always the case in all telemedicine programs), using a physician who is 
not the patient's primary care physician (also not true of all 
telemedicine activities), inability to see the patient or do a physical 
exam when needed, having the physician multitask a bit more because the 
patient is not in the room looking at the physician, poor communication 
because of accents/language issues or a host of other issues.  In theory, 
some diagnosis may be missed without telemedicine though if the patient 
with symptoms delays more because it is harder for the patient to have a 
face to face visit than a telemedicine visit.  So, there is a lot of 
unknowns that need to be discovered.   It is not clear if anyone is 
looking at this with any sort of rigor.

I have read this couple of days ago. 
It overstates benefits of telemedicine and again overstates the physician 
community response. 
As someone who dappled with the very telemedicine technology I cringe 
inside when I read some of this. 
I think "outsiders" sorry for the term, with little or no idea about the 
inner workings of the health care system have an exaggerated expectations 
of the effect of technology on health.  
If anything telemedicine should be assimilated into the already present 
structures not seen as a replacement to it. But good luck explaining that 
to Silicon Valley tech-militants who just want to disrupt health care.
