I just finished reading this book ($19.65 hardcover, or $14.31 Kindle on Amazon). Very impressive to me, the first "keep it real" account of the patient safety threats in the ill-prepared roll-out of EHR systems, even naming names, Epic and Cerner. I've always wanted to collectively address the EHR software manufacturers with a "shame on you" rant. But this book is much better than that. It is more specific and detailed, even-handed, and it describes what must happen to make things better.
There is a section on EHR patient safety as it relates to diagnosis, with stories from our own Gurpreet Dhaliwal and Gordie Schiff. And a section at the end about where all this is heading in the future.
The best "review " I can find online is actually excerpts, Wachter's own NY Times piece at http://www.nytimes.com/2015/03/22/opinion/sunday/why-health-care-tech-is-still-so-bad.html?_r=0
