I would add that improving on diagnostic accuracy and timeliness will likely require different approaches and resources than we have used in other error improvement efforts.

Dear Rob,

There are good reasons for devoting our efforts in SIDM and the Coalition to diagnostic error rather than all medical error as you suggest.


1.       Misdiagnosis is the elephant in the room of medical error.

2.       Diagnostic error has been conspicuously absent from patient safety efforts.  There has been a lot of attention and there are measures addressing other types of errors, such as ACGME cutting resident hours to reduce fatigue and error, CPOE to eliminate handwriting errors in orders, surgical checklists, handoff procedures, barcodes for patients, lockouts for medications, and so on.   There should be many more.  But until SIDM brought it to the fore, dx error was entirely unaddressed.

3.       Now that we have finally succeeded in bringing dx error to public attention, this is not the time to liquidate, abandon, dilute or lose sight of this prominent, I would say foremost, issue. We must progress beyond the attention stage to the doing-something-about-it stage. The Coalition members have signed on to that.

I too agree this is a very fine initiative. It gets things moving.

In the article entitled Errors in Medicine: Do Something Now as given was written in March 2015 and just published I thought that the whole field of the Errors in Medicine should be addressed and not just the Errors in Diagnosis as "regular errors" and diagnostic errors were so interrelated.

If the "regular error" infrastructure was faulty then would diagnoses be faulty? The other thought that I had was with the first planning sessions that the topics be triaged for importance to patients and ease of undertaking.

Perhaps the first meeting of the Coalition should decide whether Errors in Diagnosis be tackled in isolation.

The other questions are very good ones and the initial discussions will help decide the success of the venture.

Yesterday, we announced the formation of the Coalition to Improve Diagnosis.  Following is the text of the press release:

Top Medical Societies and Healthcare Organizations Commit to Address Errors in Diagnosis
Coalition to Improve Diagnosis Will Bring Attention, Action to Diagnostic Errors
WASHINGTON, DC (August 25, 2015) - More than a dozen major medical societies and healthcare organizations, including patient and consumer advocates and government partners, have joined forces
to address diagnostic errors in medicine, through the newly formed Coalition to Improve Diagnosis.

The Society to Improve Diagnosis in Medicine (SIDM) established and will lead the Coalition. The Coalition's founding member organizations represent clinicians and other healthcare professionals,
patients and families, employers, consumer advocates, researchers, policymakers and educators.
Errors in diagnosis are a system-wide issue that has received little national attention, yet they affect one in 20 patients, an estimated 12 million Americans each year. A groundbreaking report on diagnostic
error to be released by the National Academies of Sciences, Engineering, and Medicine this fall will outline the dimensions of the problem and the critical importance of addressing it.
"Physicians excel at diagnosis, but errors or delays in reaching the right diagnosis still create harm, and we can do better," said Mark L. Graber, MD, FACP, president and founder, SIDM. "The vision of our
Society-SIDM-is to ensure that diagnosis is accurate, timely and efficient. The Coalition to Improve Diagnosis will be the catalyst for changes in practice needed to realize this vision."

The following organizations are members of the Coalition:

*American Association of Nurse Practitioners,
*American Board of Internal Medicine,
*American Board of Medical Specialties,
*American College of Emergency Physicians ,
