Disappointing is putting it very politely.
At the pace set  by IOM, it will be a very long time before "medical error" and "congratulations" can justifiably appear in the same context even half as often as they have circulated in the enthusiastic comments. 
If the goal was what it turned out, they used 369 pages too many.
