In my article Errors in Medicine - Do Something Now! recently published I recommended that every doctor’s office have someone appointed who is a designated the Patient Safety Officer. Starting with communication errors and then migrating to other errors, diagnostic errors might make a difference, particularly if one of the main Medical Organization recommended that be done, with some guidance.  Might need a study to give it validity.

 think hat could make a great deal of difference.
