I think it's fine to say what you think is going on and tell the patient to
get back if the course if different than what you describe to them.  If you
have indeed considered a reasonable differential diagnosis.  Using Peter
E's list for sore throat, I think we can manage a list of possibilities
into "management categories".  The patient may have proactively answered
questions specific to symptoms and signs of the various conditions which
would winnow down the possibilities, but at the end each remaining
management category would information for the patient what steps to take if
the illness YOU think is going happening takes a different course .
