For background, I wrote the article, as a practicing Residency Program Director to help introduce diagnostic errors to physicians and educators in community hospitals.  My secondary goal was to provide a couple tools for engaging the peer review process and teaching residents.  My co-author, Julie Klein is a professor at Villanova and brought her background in ethics to help tease out various participant viewpoints, and strengthen the argument for action.  

Of interest, since writing, I have used the resident teaching scenario three more times with different resident audiences with fascinatingly similar reactions and discussions.
