Resources for diagnostic dilemmas

Out of the blue, I was contacted by my 10th grade biology teacher who asked me for help regarding his grandson. Here is the gist of what I understand: He is in his early 20’s. 4 years ago he had a severe case of infectious mono. Since then his spleen has remained mildly enlarged and becomes excruciatingly painful during recurrent episodes flu-like symptoms. He has seen academic specialists of all stripes and has been extensively imaged. As far as I can discern, he has yet to receive a satisfactory explanation for his symptoms.

I searched the literature and didn’t find anything that even remotely resembled his problem, other than a 2001 case report in Military Medicine.  I realize that the SIDM list is not the place to sort this out.  So here’s my question: Are there any online forums (or other resources) where clinicians and/or patients share diagnostic dilemmas?
