Jeff Brady from AHRQ will introduce the session, Helen Haskell will review the importance of diagnostic error for patients, Pascale Carayon will consider the problem from the perspective of human factors engineering, and I will focus on policy issues that impact on diagnostic quality and safety.  This should be an informative session, and our first opportunity to dialogue with CMS about this important issue.
