Beyond biases?

The video and paper were very interesting.  One thing that bothered my
about them is that I have always thought that anaphylactoid reactions and
ACE Inhibitor induced angioedema are different problem with different
treatments.  If I were to pick up the patient described in the case as a
paramedic, my run report wouldn't say anything about an anaphylactoid
reaction.

My question for the list is, how often do different disciplines in
healthcare have different terms to describe the same thing or who don't
agree on how things are categorized?  Also don't these difference make it
harder to identify and track errors?
