NHS and 'diagnosis'

Incredibly ‘diagnosis’ has still not even appeared on the radar screen of the NHS in the UK.
The Care Quality Commission, which regulates hospitals and primary care, has completed 3 years of inspections and has just published a report on the “State of care at NHS acute hospitals 2014-2016”. In a 102 page report the word “diagnosis” appears just once!
How is this still possible?!
