One only knows of that which is recognized and measured. Again, diagnostic
error not mentioned in the taxonomy. If missed diagnosis leads to a cascade
of dowmstream testing and treatment harm, then root cause is not at the
"sharp point"(testing and treatment), but rather gate of entry(diagnosis)

Read in conclusions:
"Advocates are not calling to prevent problems for which solutions are not
known but calling to implement known solutions to prevent all too common
problems."
