FYI, every year ECRI, a nonprofit risk management group that sells RM products/services to hospitals, issues its updated list of the Top Ten Patient Safety Issues for Healthcare Organizations.

For 2017, the new list is out.  After EHR info mgt., also near and dear to us, the next ones numbered 2-3-4 and 10 are related to diagnosis.
