Burnout

Folks:


I was invited to give the James Mills, Jr., MD lecture last year at the American College of Emergency Physicians Scientific Assembly on burnout and resiliency, which are huge issues in emergency medicine.  It is the lecture topic among many that I am most often asked to give, so healthcare systems are obviously struggling with it.


I called Christina Maslach prior to putting the talk together because I wanted her input.  I asked her how many healthcare folks had reached out to her about physician burnout, thinking it would be a very large number.  Almost sheepishly, she said, "Actually Thom, you are the first."  That was a total shock to me.


Having done an exhaustive literature review, over 90% of it is on diagnosis of burnout-only 8% has anything to do with treatment.


I guess, as the lawyers say, res ipsa loquitur.

Hi Rob,

You may find the following citations a helpful starting point. Burnout is commonly studied in occupational health psychology and consists of three constructs: emotional exhaustion, depersonalization, and feelings of inefficiency. The presence of these constructs in an individual may result in substance use like Xavier mentioned. My understanding is that burnout is prevalent in healthcare due to the constraints of the job (e.g., lots of patients, but so little time).

I would have thought that it behaved something like many of the conditions some docs get, such as drug use, alcohol use, fatigue, depression, etc. Perhaps a symptom here and there of all of these. Studies would help. Perhaps they have already been done! Is there a diagnostic description for burnout?

The AMA is focussing a lot of attention on burnout among physicians - and presumably that is also common with other HCPs.

It would seem sensible to do a few RCAs on burnout, as it is so important. This perhaps around the country and compare the results. It would be interesting, if it could be collected, to see if there were increased errors with burnout candidates (both standard and diagnostic).

Losing MDs puts more pressure on those left behind. Are the medical schools taking in more students to compensate for the losses. What are the losses?
