I spent years as Chief f Pathology and also ended up working the ER 40 hours a week. Made some mistakes but also picked up mistakes made by PCP's ad others. some complicated , some simple. Was also the Field Surgeon for the NG.

Rather than those mistakes which made it to the level of malpractice claims, how about a book about those most common diagnostic errors in speciaity practices, and then a much larger book for those in general practice


As a patient, I wonder about the PCP referring a patient with difficult case, or one misdiagnosed already, to the inappropriate specialist.  That seems more common than recognized, and can set the patient on a wrong path, frustrating to all.  The advice we patients receive is that the PCP is the one to make the referral, but being referred to the wrong 'expert' is also a diagnostic error of sorts.


In my own case of severe anemia, an ER doctor recommended I see a hemotologist, but was admitted and seen by a GI doctor, treated for 8 months for a non-existent ulcer (now found in the biopsy pathology), then suspected of having cirrhosis, needing an US to prep for a liver biopsy.  That US revealed a 10cm renal tumor, and subsequent CT, lungs full of tiny mets.  Many missed opportunities along that way to have found the tumor far earlier.
