IMPROVEDX Digest - 12 Jul 2017 to 18 Jul 2017 - Special issue (#2017-140)

I collaborated with Bob on research examining the impact of follow-up and feedback and feel his passing is a real loss to our community. His work on resilience in particular brought to our attention that maybe instead of (or in addition to)focusing on prevention of error, we need to work on ways to recover from error that minimize harm. He was one of the few people in the diagnostic error community with that focus and he will be missed.

I concur with Mark that losing Bob Wears is a huge loss to the patient safety and diagnosis error community.

He was a true breakthrough thinker, who always strived and more often than not succeeded in pushing the envelope for critically thinking through the difficult issues in diagnosis.
He was particularly sensitive to the issues of hindsight bias that plague much of our work, and I will miss both his admonitions to avoid this tendency, as well as suggestions of how to overcome it .

I attached a brief editorial I had the privilege to write with him more than a decade ago.
