EHR-Related claims on the rise

Thanks for sending this on. I am particularly annoyed by the "note bloat,"
in which a visit note from a consultant cuts and pastes everything from
his/her initial comprehensive H+P to every imaging test that was ever done.
It appears to be done to allow billing at a higher level. The major
downside is that it is impossible to figure out what was done at the most
recent visit.
