Actually, there is a form of litigation remediation for those who share errors for analysis. That is the Patient Safety and Quality Improvement Act of 2005, establishing PSOs. Some of the physician specialty-oriented PSOs are looking at diagnostic error—primarily that’s occurring in the diagnostic specialties. Here’s a list of the current PSOs: https://pso.ahrq.gov/listed.

So the mechanism is already there—a new PSO could be established focusing on diagnostic errors, and take submissions from all providers through a web interface (some PSOs work like this), or an already-established PSO can collect diagnostic errors through its relationships with specific providers (and some do already).

We do collect everything at CHPSO, but the standard taxonomies we use make it hard to pull out the diagnostic errors. We’ve pulled out specific diagnostic issues on occasion for specific purposes, but haven’t done a generalized review of our data. This would be a good research project to look for AHRQ funding—they’ve emphasized work with PSOs in some of their grant opportunity announcements, and this would be a natural for that sort of collaboration.,

This is all so good - a kudos to SIDM.

However, I still maintain that you may be on the wrong track for SIDM to get something big accomplished.

It seems to me that unless all (or even some) hospitals and doctors are completely open to divulging their errors so that they can be analyzed, nothing significant will happen. And for this to happen, I believe, it means that there needs to be be some form of litigation remediation.

So why not work on that. I realize it is an Everest of a task.  But would not this be better than jogging a long not knowing how many lives we are saving each year when we focus on diagnosis alone. Collaboration with other societies (both National and International) might be a way forward.

I have suggested that SIDM work on seeing that all the diagnostic tests we use be improved so that presumably the error rate is less. This to support the diagnostic research area. What do our diagnosis mean if the tests used have their own error rate and some significant?  Do we even have a list of all tests done with their error rates? Could we focus on the worst there?

Another thought is getting one or two or more hospitals to become open with errors (with protections - litigation and perhaps coded information to a research body) and use these as research labs with regards to errors, including diagnostic.

The question is, do we need to think BIG?

As a small point can anyone tell me if warm hands or cold hands alter an oximeter reading? Is hand temperature one of the recommendations for taking an oximeter reading?
