Virtual patients to study and improve clinical reasoning

I used to teach microbiology to nursing students at both 2 and 4 year institutions. At the end, the syllabus would call for an unknown - students would get a culture and then inoculate all the media types we had studied in order to arrive at an ID. I wanted to make the class more clinical and replicate what they would actually do. So, since nurses rarely (if ever) inoculate and read tests, I gave them an initial culture and a detailed H&P. Based on the clinical presentation, they would first order microbiology and other clinical tests to determine what was potentially wrong. They had a pick list of 20 potential organisms (including viruses, which we couldn't culture) to start with. I would email them the results within a clinically relevant time frame.
They could then order other tests. More results given to them. Points were awarded for appropriate tests and correct ID. Points were subtracted for inappropriate testing or incorrect use of a diagnostic algorithm.
Students loved it because it replicated what they do in their profession.
