Important  quote. "We should be thinking about how we recognize other people's errors.”

Are there any ranked lists of the commonest errors in medicine in the USA?

Are the RCAs of sentinel events, the combined insurance industry law suit data, and any other readily available data easily accessed? If not why not?

Would this data help us reduce errors?

Is there any International data available - could that help the USA?
